He provided medical counsel and was present during Beau’s craniotomy. In 2021, O’Connor described Biden as a “healthy, vigorous, 77-year-old male” fit for the presidency. He diagnosed Biden with COVID-19 in July 2022, prescribing Paxlovid, and communicated updates via memos rather than public briefings, which drew some criticism.

Controversies emerged, particularly after Biden’s June 2024 debate performance, raising questions about his health. House Oversight Committee Chair James Comer requested O’Connor testify about Biden’s health and alleged ties to Biden family business dealings, citing a $200,000 payment linked to a healthcare program with Americore. Some X posts, including from political figures, have accused O’Connor of concealing Biden’s health issues, including a possible cancer diagnosis, and engaging in misconduct, though these claims lack verified evidence and remain speculative. O’Connor reportedly pleaded the Fifth during a 2025 House hearing on Biden’s cognitive health.
