President Joe Biden’s State Department has launched an internal investigation into the Biden administration’s chaotic evacuation from Afghanistan, Fox News reports.
Inspector General Diana Shaw told Congress the State Department will begin “several oversight projects” to determine what went wrong at the end of America’s 20-year mission.

The new investigation will look into the evacuation, the Special Immigrant Visa (SIV) program, and the resettlement of thousands of Afghan refugees, according to an Oct. 15 memo to State Department Secretary Antony Blinken.
Shaw cited the “elevated interest in this work by Congress” in a letter to the House and Senate Intelligence and Foreign Affairs committees.
The Biden administration, and the State Department, in particular, was criticized for failing to prepare to evacuate Americans and Afghan allies with Special Immigrant Visas (SIV) before a rapid Taliban takeover in August forced the United States to flee the country in a scramble.
When the smoke cleared, hundreds of Americans were left behind, and 13 American soldiers had been killed in a terror attack. A retaliatory strike that the Biden administration said took out terrorists was later found, according to The New York Times, to have killed innocent Afghan children.
The debacle dealt a blow to American prestige and led to a fresh refugee crisis with security risks, as thousands of desperate unvetted Afghans were frantically packed onto airplanes to be flown out of Kabul.
The Pentagon is said to be reviewing the deadly airstrike on Afghan civilians and its screening process for Afghan refugees, the Washington Examiner reported.
America’s top generals went before Congress last month to testify about the war, but they emerged largely unscathed despite calls for their resignation from numerous Republican lawmakers.
For his part, Biden has trumpeted the evacuation as an “extraordinary success” that rescued Americans and Afghan allies despite incredible odds, although the majority of evacuees fall into neither category.
While accountability for Biden’s debacle remains elusive, some have called for a wider war commission to investigate the overall course of the failed 20-year occupation, which transformed from a counter-terrorism operation into a doomed effort to Westernize Afghanistan.
