An explosive undercover investigation by citizen journalism group Sage Ops has revealed several New York nonprofits providing gender transition services to minors without parental knowledge or consent, while receiving substantial federal funding for these operations.
The investigation targeted organizations including the Ali Forney Center, Care for the Homeless and the Office of Children and Family Services, documenting how these groups work together to provide housing and medical services to minors seeking gender transition services outside of parental oversight.
In hidden camera footage, Nadia S., a trans-identifying Health Services Coordinator at Ali Forney Center, explained their reporting policies.
“We’re mandated reporters, we’re mandated to call ACS. We don’t contact the parents. We call the state central registry with ACS,” she said, referring to the Administration for Children’s Services.
Staff at the center consider parental opposition to a child’s gender transition as potential abuse, according to Nadia.
“Homophobia, transphobia – all of that is recognized as a legitimate form of abuse or neglect in New York state,” she stated.
“But sometimes we have to do some advocacy with [Administration for Children’s Services] around that… with Children’s Protective Services. They would do something called a ‘Family Crisis Contract.’”
The investigation found that minors are quickly placed in confidential housing locations where parents cannot locate them.
“Folks, especially those who are minors, generally will get placed into our housing sites quicker because we prioritize minors. All of those housing sites are confidential locations,” Nadia explained, adding that “The medical part can happen relatively swiftly.”
She emphasized the secretive nature of these arrangements, stating, “The locations are private. We’ll never force somebody to have contact with parents.”
Maria Mendoza, a licensed clinical social worker at the Office of Children and Family Services, confirmed to undercover investigators that CPS workers actively assist minors in accessing transition services.
“A 16-year-old child can get the care she wants – absolutely. She’s 16 so she’s allowed to make her own choices,” Mendoza said, adding that “The CPS worker can navigate the child through the process. They’ll investigate it.”
The investigation extended to the Institute for Family Health, a partner of the Ali Forney Center, where Associate Regional Director Lindarey Gonzalez disclosed that they provide gender-affirming care without requiring parental consent or involvement.
“The child can come by themselves. Since it involves those types of services, they don’t have to inform the parent,” Gonzalez said.
She contrasted this with regular medical services, noting that “If they’re coming for a regular physical, technically, they do have to bring their parents because they’re billed under their parents’ insurance.”
She added that if minors don’t want to use their parents’ insurance, “we have other resources for them.”
Jonathan Ramos, Chief Program Officer at Care for the Homeless, confirmed to investigators that “In New York state, we can start care without the parental consent,” noting that minors are “eligible for Medicaid, CHIP” and can “start the process” of gender transition using these government programs.
Financial records obtained during the investigation show these organizations have received significant federal funding.
Since 2020, the network has reportedly received over $180 million from the Department of Health and Human Services, with nearly $10 million awarded in 2025 alone, plus approximately $150 million in Medicaid reimbursements.
Recent executive orders signed by President Trump in January 2025 aim to withdraw federal funding from institutions providing gender transition treatments to individuals under 19.
Some medical facilities, including NYU Langone and Children’s National, have already suspended such services for minors to avoid losing federal funding.
Gonzalez expressed concern about these policy changes during the undercover recording.
“The biggest risk we could face is if the Trump administration decides to withhold the federal piece of Medicaid dollars,” she said, mentioning Elon Musk’s role in reducing federal spending and his public opposition to gender transition procedures for minors.
